The physician team at Puget Sound Orthopaedics utilizes a secure, online New Patient Appointment portal called ~~ Simple Admit.
During your initial appointment call with a member of our Front Desk staff, you will be provided a unique Simple Admit Password which will be required to begin the online appointment process:

Simple Admit Registration Process:
Your medical facility must know your health information and medical history prior to your procedure. Filling out the online form is easy, secure, and essential for you to receive the best care possible.
Please be forthcoming with your responses.
All of your medical information is confidential.
The time it takes to complete the form is dependent on your personal medical history. Typically, a person who is healthy and not taking many medications should expect to spend 15 minutes completing the form. Someone with a more complicated medical history or list of medications will take longer.
Upon receipt of your information, a staff member or nurse may call you for further clarification if necessary.
NOTE – You do not have to fill out a form again if you are returning to the same medical facility and have already completed this online form prior to a previous visit. A nurse or staff member will call you to discuss any changes in your medical history since your last visit.

Before you begin the online admissions form, please make sure that you have the following information available, as it may be needed to complete your form:
Prescription Medications – names and dosage of each one you take.
Over-the-counter Medications – names and dosage of each one you take regularly.
Vitamin and herbal medications – names and dosage of any that you take.
Allergies/Sensitivites you have, and the allergic reaction(s) they cause.
Previous surgeries, and the year they were performed.
Medical Insurance – insurance company, policy number, ID number, etc.

What To Bring To Your First Appointment
Your insurance card and photo ID
A referral from your primary care doctor, if required by insurance
Any X-rays, MRI or Bone Scan films you have.
In addition to any X-rays or MRIs you have had taken, your doctor will also require any prior treatment records relating to the problem you are being seen for.
Arrive at least 15 minutes early so that we may check you in and verify your information. If you need to cancel or reschedule an appointment, please call 24 hours in advance.
Dress comfortably for your appointment in loose fitting clothing that don’t have metal, such as buttons and sequins. Depending on what body part you are being seen for it is beneficial to wear a certain type of clothing:
Upper Extremity ~ loose fitting tank top or sleeveless shirt.
Lower Extremity ~ loose fitting pants or shorts. Please avoid wearing jeans.
Co-payments are due at the time of service. For your convenience, we accept cash, checks, Visa, MasterCard and American Express.
We require at least 24 hours’ notice for any prescription refills. Please call your pharmacy and ask them to fax us a refill request to:

Your first appointment will begin with taking a detailed medical history and conducting a physical examination. The history gives your physician an idea about the type of symptoms or pain you may be experiencing, your past medical and surgical history, and medications that you are currently taking. We may take x-rays of you in the clinic, if you have not brought x-ray images with you.
We participate in most insurance plans, Medicare on assignment, and most L&I and Self Insured Worker’s Compensation Claims.
