We suggest that you talk to your primary care physician about getting a referral, or call us directly at 253-582-7257 to schedule your first consultation with one of our orthopedic surgeons. You can also request an appointment online once you are an established patient with us.

What to bring to your first appointment:
Your insurance card
A referral from your primary care doctor, if required by insurance
The completed New Patient Forms.
Any X-rays, MRI or Bone Scan films you have.

Please arrive at least 15 minutes early so that we may register you. If you need to cancel or reschedule an appointment, please call 24 hours in advance.

Second Opinion
In addition to any X-rays or MRIs you have had taken, your doctor will also require any prior treatment records relating to the problem you are being seen for.

Insurance Information
We participate in most insurance plans, Medicare on assignment, and most L&I and Self Insured Worker's Compensation Claims.

Prescriptions
We require at least 24 hours notice for any prescription refills. Please call your pharmacy and ask them to fax us a refill request to 253-582-1617.

Insurance / Disability Forms
Please allow 7 to 10 working days to process any insurance and/or disability forms that you may have filled out.

Billing
Co-payments are due at the time of service. For your convenience, we accept cash, checks, Visa, MasterCard and American Express.
