Patient Portal – MyChart

Our patient portal is a tool that will allow you to view your own medical records and communicate with our office and your provider’s staff quickly and in a secure manner.

Patient Registration – Phreesia 

Proliance Puget Sound Orthopaedics (PSO) utilizes a secure, online patient appointment portal called Phreesia. The Phreesia portal allows you to fill out your health information and medical history prior to your appointment or procedure. Your Phreesia link will be delivered to you via email or text message 5 days and 3 days before the date of your appointment or procedure. You can fill out your information in Phreesia at home from your own device or from a tablet provided in our office on the day of your appointment. Filling out the online form is easy, secure and essential for you to receive the best care possible.

Insurance and Authorizations

Proliance Puget Sound Orthopaedics participates in most insurance plans, Medicare on assignment, and most Labor and Industries and Workers Compensation claims.

Preparing for Visit

Your first appointment will begin with going over your medical history pre-registration form and conducting a physical examination. The history gives your physician an idea about the type of symptoms or pain you may be experiencing, your past medical and surgical history and medications that you are currently taking.
