Request an Appointment
Requesting an appointment through MyChart is for established patients only.
If you are a new patient and would like to request an appointment, fill out the request an appointment form on the website and a representative will contact you within 48 hours.
View Medical Visit Summaries
View notes from your clinic visit, including follow-up care instructions.
Request Prescription Refills
We require 48 business hours’ notice for all prescription refills. All medication refills that come in after 12:00 pm on Friday will not be addressed until the following Monday.
IN CASE OF EMERGENCY
The MyChart patient portal should not be used in urgent situations. Please call your physician at 253.387.1472 if the situation requires immediate attention, or dial 911 for a medical emergency.
ENROLLING WITH MYCHART
The first step is to set up your individual account. All Puget Sound Orthopaedics patients are eligible to sign up for MyChart. To do so you’ll need the activation code from your enrollment letter. If you have not yet received this letter, please call our office at 253.387.1472 to receive your code. If you have seen a CHI Franciscan provider, then you may already have an account.
To sign up:
- Go to the MyChart Login Page.
- Click ‘Sign Up Now’ under the ‘New User?’ box on the right side of the screen.
- Fill out the 3-step identification process and then click ‘Next.’
